Patient Information

Your visit to Dr. Coovadia’s rooms starts with meeting Ms Melissa Du Plessis. She will inform you of what is required prior to the appointment and will ask you to complete a patient information sheet, which will list all your relevant details. To minimize waiting time, this form can be completed electronically, and e-mailed to her prior to the consultation. Please bring along your referral letter (if you have one) and all previous medical investigations that you have with you – this can be emailed to Anna Marie prior to your appointment, so that Dr. Coovadia can review your previous results prior to your consultation.

Dr. Coovadia is contracted to selected medical aid plans. Please feel free to enquire if Dr. Coovadia is a designated service provider for your medical plan. Should your plan not fall within the contracted medical aid plans – you will be billed at the Discovery Classic Rate.

It remains the responsibility of the patient to obtain authorization for any procedures that the patient may undergo. Melissa will be glad to help, should this be required.
